1. Purpose of This Policy

Function Focus Therapy is committed to protecting the privacy and confidentiality of all personal

and health information we collect in the course of providing occupational therapy and related

NDIS services. This policy outlines how we handle, store, use, and disclose personal

information in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles

(APPs), and the NDIS Code of Conduct.

2. Information We Collect

We collect and hold information necessary to provide high-quality occupational therapy and

NDIS-related services. This may include personal details, health information, NDIS plan details,

information about other allied health professionals involved in your care, progress notes,

assessments, reports, consent forms, and correspondence related to service provision.

3. How We Collect Information

Information is collected directly from participants, their guardians, or authorised representatives

through online referral forms (via JotForm), clinical assessments, emails, and third-party

referrals. We may also collect information from our website contact or feedback forms.

4. How We Store and Protect Information

We take reasonable steps to protect personal information from loss, misuse, unauthorised

access, or disclosure. Information is stored electronically via Microsoft OneDrive, password

protected laptops, and locked filing cabinets. Access is restricted to authorised personnel only.

5. Retention and Disposal of Information

Client records are retained for a minimum of 7 years after last contact, or until age 25 for

minors, in accordance with professional and legal standards. Records are securely destroyed or

deleted thereafter.

6. Use and Disclosure of Information

Information is used only for purposes directly related to service delivery, coordination, reporting,

and administrative functions. It will not be disclosed for any secondary purpose without consent,

unless required by law.

7. Third-Party Systems and Overseas Data Storage

Function Focus Therapy uses secure platforms such as Microsoft OneDrive and JotForm. While

these primarily store data in Australia, backup servers may be located overseas, managed

under strict security and encryption standards.

8. Consent to Share Information

Participants complete a Privacy and Consent Form to specify who we may share information

with, such as Support Coordinators, Plan Managers, NDIS, GPs, or family members, as

authorised. Information will not be disclosed without consent unless required by law.

9. Access and Correction of Information

Participants may request access to or correction of their information by emailing

hello@functionfocustherapy.com.au. We will respond within 24 hours and act promptly upon

verification of identity.

10. Privacy Complaints

Concerns about privacy can be directed to hello@functionfocustherapy.com.au. If unresolved,

complaints may be referred to the Office of the Australian Information Commissioner (OAIC) via

www.oaic.gov.au or 1300 363 992.

11. Website and Cookies

Our website may use cookies or analytics to collect non-identifiable usage data to improve

functionality. Users can disable cookies through their browser. Data submitted through forms is

used solely for service or response purposes.

12. Staff Confidentiality and Access

All staff, contractors, and students are bound by confidentiality agreements and the NDIS Code

of Conduct. Client information access is restricted to authorised team members only.
