Function Focus Therapy is committed to protecting the privacy and confidentiality of all information we collect in the course of providing occupational therapy and related NDIS services. This policy outlines how we handle, store, use, and disclose personal information in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs), and the NDIS Code of Conduct.

We collect and hold information necessary to provide high-quality occupational therapy and NDIS-related services. This may include personal details, health information, NDIS plan details, information about other allied health professionals involved in your care, progress notes, assessments, reports, consent forms, and correspondence related to service provision.

3. How We Collect Information

Information is collected directly from participants, their guardians, or authorised representatives through our online referral form, clinical assessments, emails, and third-party referrals. We may also collect information from our website contact or feedback forms.

4. How We Store and Protect Information

We take reasonable steps to protect personal information from loss, misuse, unauthorised access, modification, or disclosure.

Personal information is stored securely through our electronic case management and document management systems, password-protected devices, and, where applicable, secure physical filing systems. Access to personal information is restricted to authorised personnel who require access for the purpose of providing services or carrying out their role.

We use appropriate security measures, including password protection and access controls, to help protect information from unauthorised access or disclosure. Physical records, where retained, are stored in secure and locked locations.

5. Retention and Disposal of Information

Client records are retained for a minimum of 7 years after last contact, or until age 25 for minors, in accordance with professional and legal standards. Records are securely destroyed or deleted thereafter.

6. Use and Disclosure of Information

Information is used only for purposes directly related to service delivery, coordination, reporting, and administrative functions. It will not be disclosed for any secondary purpose without consent, unless required by law.

7. Third-Party Systems and Overseas Data Storage

Function Focus Therapy uses secure third-party platforms to support the delivery and administration of services, including Splose for client and clinical records, Xero for accounting and financial management, and Microsoft OneDrive for secure document storage.

These platforms may store or process information on servers located in Australia or overseas. Function Focus Therapy selects reputable service providers that use appropriate security measures, including encryption and access controls, to protect information. Access to these systems is restricted to authorised personnel and managed in accordance with Function Focus Therapy’s privacy and information security procedures.

8. Consent to Share Information

We will discuss with you who we may need to contact or share information with as part of providing our services. This may include Support Coordinators, Plan Managers, NDIS, GPs, other health professionals, support providers, or family members, where relevant to your care.

We will seek your consent before contacting or sharing information with third parties. We will not disclose your personal information without your consent unless disclosure is required or authorised by law.

9. Access and Correction of Information

Participants may request access to or correction of their information by emailing

We will respond within 24 hours and act promptly upon verification of identity.
